Free Senior Advocacy and Transitional Care Services

Moving from one’s home to an extended care community can be overwhelming for the individual and the family. There are serious health, financial, legal, and logistical concerns that can make this process incredibly stressful and complicated. That’s why Natural Healthcare Center is proud to connect patients, families, and friends with free senior living support services through A Rae of Hope.

Jennifer Rae and her family of senior advocates at A Rae of Hope will work in your best interests, offering objective, unbiased guidance based on the specific needs and condition of you or your loved one. Jennifer also has a trusted network of medical providers, legal service providers, financial consultants, realtors, movers, home cleanup services, and other resources to support your journey.

There is no cost for these services to seniors or their adult children, regardless of income. A Rae of Hope receives a referral fee from the senior living community or skilled nursing care facility once you’ve chosen where you or your loved one will live.

How Senior Living Support Services Work

You’ll schedule an initial consultation with Jennifer Rae, Advanced Practice Nurse (APN) at Natural Healthcare Center and owner of A Rae of Hope. Jennifer will learn about your health history, challenges, concerns, and objectives, and answer any questions you may have. If you decide to move forward, Jennifer will guide you through the transitional care process that includes but is not limited to:

Facility tours
Health evaluation, medication assessment, and lab testing
Coordination of care
Rate negotiation
Timely completion and submission of paperwork
Thorough review of your options

The purpose of these services is to help you make decisions with confidence and peace of mind so you can look forward to a safe, fulfilling life.

Meet Jennifer Rae

As an APN at Natural Healthcare Center, Jennifer Rae, AGNP-BC, MSN-Ed provides a variety of services ranging from trigger point injections and pain management to behavioral healthcare and medication management. Jennifer also applies her extensive experience in geriatrics and adult medicine as owner of A Rae of Hope, helping seniors and adult children navigate the complex “next steps” of moving to an extended care community. Jennifer is one of the only board-certified nurse practitioners offering these services in New Jersey.

A Message For Our Patients

On Saturday, March 21, 2020 New Jersey Governor Phil Murphy held a press conference in which additional measures were enacted to curtail the spread of Covid-19 and issued Executive Order 108. This Order states in part that “any health facilities that provide medically necessary or therapeutic services” can remain open. The Order further states “Nothing in this Order shall be construed to limit, prohibit or restrict in any way the provision of health care or medical services to members of the public.”

We are continuing to take all precautions to ensure that our patients and staff stay safe during this unprecedented time:

We are staggering our appointment times to promote social distancing.
All rooms and equipment will be disinfected and cleaned with medical grade disinfectant immediately before and after each patient encounter.
Our clinicians will wear gloves for your treatment upon request.

We ask that if you have any cold or flu-like symptoms, including a cough or fever, or have recently traveled to an area affected by the coronavirus, that you do not come in for your appointment but rather call us to reschedule.

If you do have a fever or other flu-like symptoms, or have possibly been in contact with someone who has been diagnosed with coronavirus, please self-quarantine and contact your primary care doctor as soon as possible.
